For testing, message your PCM via the MHS GENESIS Patient Portal. Be sure to include details, like the type of test you’re looking for and why you need it.

You can also find off-base testing information at https://www.elpasocountyhealth.org/covid-19-testing-information.

Periodic Health Assessments (PHA)

To initiate your PHA, please click the PHAQ Link located in the PHA Column of your IMR Page (If you're not due for your PHA, you will not be able to start a new PHAQ without the MTF unlocking it).

Once you complete the PHAQ, you will be contacted by the Base Operational Medicine Clinic (BOMC) for your Mental Health Assessment and any required in-person appointments.

If the patient’s sponsor is at USAFA and you need additional assistance, contact the EFMP office at 719-333-5991.

If the patient’s sponsor is at Peterson/Schriever, contact their EFMP office at 719-556-1392. If the patient’s sponsor is at Fort Carson, contact their EFMP office at 719-526-7805.
