Health Management

What We Do

 We help minimize complications in patients with chronic diseases and help lessen the long-term usage of health care resources for patients with conditions such as:

  • Pre-diabetes
  • Diabetes
  • Hyperlipidemia (High Cholesterol)
  • Hypertension (High Blood Pressure)
  • Individual Health Management Counseling - We provide individualized Counseling to 10thMedical Group enrollees.
Additionally, we provide health education and advocacy by helping patients follow their treatment plans and gain control over and become more proactive in their healthcare.

We serve TRICARE Prime and TRICARE Plus Enrollees at the United States Air Force Academy/10th Medical Group.

Individual Counseling

Group Counseling

Diabetes Group Class

Patients diagnosed with diabetes are encouraged to attend our multi-disciplinary diabetes management class.

It's held on the first Tuesday of each month from 8:00 a.m. to 12:30 p.m. in the conference room on the second floor of the main clinic. The conference room is immediately in front as you exit the elevators.

Pre-Diabetes Group Class

Patients diagnosed with pre-diabetes are encouraged to attend our class which focuses on lifestyle modification and nutrition interventions.

It's held on the second Tuesday of each month from 8-10 a.m. in the conference room on the second floor of the main clinic. The conference room is immediately in front as you exit the elevators.


Total Body Wellness Class

Our Total Body Wellness Class offers comprehensive wellness plan with education by:

  • Registered Dietitian
  • Registered Nurses
  • Exercise Physiologist
  • Clinical Pharmacist
  • Behavioral Health Provider

Class is held each Wednesday from 3:30 p.m. to 4:30 p.m. in the main clinic building, second floor classroom (room 2050) We provide free fitness and nutrition gear to program participants.
