What We Do

 We help minimize complications in patients with chronic diseases and help lessen the long-term usage of health care resources for patients with conditions such as:

  • Pre-diabetes
  • Diabetes
  • Hyperlipidemia (High Cholesterol)
  • Hypertension (High Blood Pressure)
  • Individual Health Management Counseling - We provide individualized Counseling to 10thMedical Group enrollees.
Additionally, we provide health education and advocacy by helping patients follow their treatment plans and gain control over and become more proactive in their healthcare.

Who We Serve

We serve TRICARE Prime and TRICARE Plus Enrollees at the United States Air Force Academy/10th Medical Group.
