How to Make an Appointment

Routine/Annual Mammograms

Must be 40 or older and must be at least 365 days since last mammogram. To schedule a routine or annual mammogram, contact Patient Access Services at 719-524-2273.

Diagnostic Exams/Breast Biopsies

To schedule a diagnostic exam or breast biopsy, contact the clinic directly at 719-333-5438.

Mammograms Ordered by Off-Base Provider

For mammograms ordered by an off-base provider, the provider must fax the order to 719-333-0507.
